The NCLEX (National Council Licensure Examination) is a crucial step for nursing graduates seeking to become licensed nurses in the United States.
To ensure the competence and safety of future nurses, the NCLEX evaluates their ability to apply the knowledge and skills necessary for entry-level nursing practice. Understanding the NCLEX test plan or NCLEX blueprint is essential for nursing students preparing for this important examination.
In this NCLEX test plan, we outline the content and structure of the NCLEX. It helps you understand what to expect and prepares you to demonstrate your proficiency in various nursing concepts and skills.
Let’s dive in.
Changes in the NCLEX test plan for 2023
The NCLEX test plan undergoes periodic revisions to ensure its alignment with current nursing practice and the evolving healthcare landscape. In April 2023, several changes were implemented to reflect the latest advancements in the field. These revisions aim to better assess a candidate's ability to provide safe and effective nursing care.
The changes in the NCLEX test plan for 2023 include updates to the content distribution and the addition of new emphasis areas. These modifications of NGN NCLEX address emerging healthcare trends and place a stronger emphasis on areas critical to nursing practice.
NCLEX test plan content
The NCLEX test plan serves as a framework for organizing and categorizing the content areas covered in the nursing license exam. Both the NCLEX-RN test plan and the NCLEX-PN test plan consist of three main components:
1. Client needs categories
The client needs categories are fundamental areas of nursing practice that form the basis for the NCLEX test questions. They encompass four key domains:
#1 Safe and effective care environment:
This category assesses a nurse's ability to provide a safe and therapeutic environment for patients, ensuring their well-being and privacy.
It has two subcategories:
Management of care for RNs - 15-21% / Coordinated care for PNs - 18-24%
Safety and infection control for both RNs and PNs - 10-16%
Topics covered in this category include client advocacy, collaboration with the healthcare team, adherence to ethics, prioritization of care tasks, supervision of client care, and maintaining confidentiality.
#2 Health promotion and maintenance:
6-12% for both RNs and PNs.
This section focuses on promoting wellness and preventing and detecting illness. Key topics include health education, patient advocacy, disease prevention, and health promotion across the lifespan. Mastering this area will demonstrate your ability to provide comprehensive care and contribute to the overall well-being of your patients.
#3 Psychosocial integrity:
6-12% for RNs.
9-15% for PNs.
This category focuses on the nurse's ability to provide emotional and psychological support to patients, recognizing and addressing their mental health needs.
This category covers topics such as crisis intervention, abuse and neglect, behavioral management, cultural sensitivity, grief and loss, and end-of-life concerns.
#4 Physiological integrity:
Physiological integrity examines the nurse's comprehension of physiological and pathophysiological processes, including pharmacology, nutrition, and other aspects of physical health. It helps nurses to gain the ability to provide competent care, including basic care and comfort, pharmacological therapies, and management of acute and chronic conditions.
This category includes four subcategories for both RNs and PNs:
Basic care and comfort, which focuses on providing comfort and hygiene. 6-12% for RNs / 7-13% for PNs.
Pharmacological and parenteral therapies, which assess knowledge of medication administration. 13-19% for RNs / 10-16% for PNs.
Reduction of risk potential, which involves identifying risks and preventing complications. 9-15% for both RNs and PNs.
Physiological adaptation, which covers understanding physiological changes and providing appropriate care. 11-17% for RNs / 7-13% for PNs.
Each of the above-mentioned categories is further divided into subcategories that cover specific client needs. To pass the NCLEX exam, nurses should comprehensively understand nursing knowledge and skills across all categories. NCLEX study guides and NCLEX practice questions can be helpful resources for preparation.
2. Integrated processes
The integrated processes in the NCLEX test plan emphasize the cognitive skills and critical thinking abilities necessary for nursing practice. They are woven throughout the client needs categories and include the following areas:
#1 Caring:
This process highlights the nurse's ability to provide compassionate, patient-centered care, ensuring the holistic well-being of individuals.
The nurse and client should be able to interact with mutual respect and trust. In this collaborative environment, the nurse should provide encouragement, hope, support, and compassion to help achieve desired outcomes.
#2 Clinical judgment:
Clinical judgment is the observed outcome of critical thinking and decision-making. It is an iterative process that utilizes nursing knowledge to observe, assess, identify prioritized client concerns, and generate evidence-based solutions for safe client care. We will further explain the steps of this part.
#3 Nursing process or clinical problem:
It is a scientific, clinical reasoning approach to client care that includes assessment, analysis, planning, implementation, and evaluation.
#4 Communication and documentation:
It assesses verbal and nonverbal interactions between the nurse, client, significant others, and the healthcare team. Events and activities associated with client care are recorded in written or electronic records, demonstrating adherence to practice standards and accountability.
#5 Culture and spirituality:
It involves the interaction between the nurse and the client (individual, family, or group), including significant others and populations. It recognizes and considers the client's reported, self-identified preferences, individual uniqueness, the applicable standard of care, and legal considerations.
#6 Teaching and learning:
This process evaluates the nurse's ability to educate and empower patients, their families, and the community about healthcare practices, disease management, and health promotion.
3. Clinical judgment
Clinical judgment is among the other NCLEX categories. Nurses actively participate in a repetitive and multi-step process that utilizes nursing knowledge to carefully observe and evaluate presenting situations. Through this process, they identify and prioritize client concerns and generate evidence-based solutions in order to provide safe care to the client.
The NCLEX content of clinical judgment can be presented either as a nursing case study or as an individual item on its own. A case study consists of six items that are related to the same client scenario and provide unfolding client information through the following steps:
#1 Recognizing cues:
The nurse identifies relevant and important information from various sources, such as the client's medical history and vital signs.
#2 Analyzing cues:
The nurse organizes and connects the recognized cues to the client's clinical presentation.
#3 Prioritizing hypotheses:
The nurse evaluates and prioritizes hypotheses based on factors such as urgency, likelihood, risk, difficulty, time constraints, and more.
#4 Generating solutions:
The nurse identifies expected outcomes and uses the hypotheses to define a set of interventions that will contribute to achieving those outcomes.
#5 Taking action:
The nurse implements the solutions that address the highest priority concern.
#6 Evaluating outcomes:
The nurse compares the observed outcomes with the expected outcomes to assess the effectiveness of the interventions.
NCLEX Test format
The NCLEX is a computerized adaptive test (CAT), which means that the difficulty level of the questions adapts based on the test-takers' responses. The exam primarily consists of multiple-choice questions (MCQs) with four answer options. However, alternate item formats, such as select-all-that-apply, ordered response, drag-and-drop, and fill-in-the-blank, may also be included.
The test duration for NCLEX-RN students is 6 hours for answering 75-265 questions, and the test duration for NCLEX-PN students is 5 hours for answering 85-205 questions. The actual number of questions administered depends on the candidate's performance.
